This brand-new project for east to west started in July 2024 partnering with ProCare Health and the Matrix Trust. To provide social prescribing to six Guildford surgeries.

Social Prescribing works closely with General Practitioners (GPs) linking young people with no-medical support for issues such as low mood, anxiety/worries, relationship difficulties and self harm behaviours which does not meet NHS Clinical Criteria for Support. Working predominantly with young people aged 11-18 east to west provide a Relational Support Worker within each GP for minimum of one day on a fortnightly basis.

During this time the east to west Relational Support Worker provides 1:1 sessions building a positive relationship whilst responding to the needs being presented. The east to west Relational Support Worker will use a variety of approaches including mentoring, signposting and coaching to challenge the young person to think, learn and reflect.

The aim is to help the young person, even in a time of need to take greater ownership of the way they respond in situations and empower them to move forward.
