At east to west, we have a proven track record of achieving successful outcomes across our projects.

Central to this success is our relational approach — a way of working that consistently delivers positive, lasting change. By building strong, trusting relationships, we empower children, young people, and parents to take greater ownership of their journeys, setting meaningful goals that help them thrive.

Our team works directly within each project, embedding ourselves in its daily life and infrastructure to ensure our support meets the unique needs of every individual.

Supporting Schools

Our team of relational professionals works collaboratively to support the school community, promoting the wellbeing, growth, and engagement of children, young people, and their families. Using a whole-school relational approach, we foster positive relationships, strengthen communication, address underlying issues, and empower students and families to thrive socially, emotionally, and academically, ensuring every child has the support they need to succeed.

Relational Support Workers

A Relational Support Worker helps children and young people develop greater ownership over how they respond to different situations, empowering them to make positive changes and move forward. Through approaches such as solution-focused coaching and CBT-informed practice, they support individuals and groups in working toward shared goals that promote wellbeing and growth. Embedded within the school community, the Relational Support Worker contributes to a whole-school relational approach, strengthening the school’s overall capacity to support and nurture its students.

The east to west hospital project provides compassionate pastoral care and emotional support for young people aged 7–21 and their families who have attempted suicide, self-harmed, or are experiencing an emotional crisis.

Operating within St Peter’s Hospital, our team works across clinical areas where young people may present with self-harm–related concerns. This includes Paediatrics, A&E, and Ash Ward, and may extend to other wards where patients aged 7–21 are admitted.

This is a non-clinical service designed to complement, not replace, the medical and psychological care provided by hospital staff.

For more information on joining our dedicated team of volunteers head to the volunteers page

The team also offer follow up support with young people and their families in their local community after discharge from St Peter’s. These one-to-one sessions often take place in relaxed, familiar spaces such as local coffee shops and when appropriate, we may also connect young people with our Relational Support Workers based in schools for ongoing care.

Supporting GP Surgeries and Primary Care Networks

east to west partner with GP surgeries and Primary Care Networks to provide relational, one-to-one support for young

people aged 11-18 and their families.

Our support focuses on non-medical help

on challenges such as anxiety and

worries, grief and bereavement,

relationship difficulties, and self-harm

behaviours that do not meet the NHS

clinical threshold for intervention.

Through relational support workers and

counsellors, we offer up to eight sessions

that focus on building trust and

responding to the young person's needs.

Using approaches such as mentoring,

signposting, and coaching, we help them to

think, learn, and reflect, encouraging personal

growth and resilience.

Our goal is to empower young people to take greater ownership of how they respond to life's challenges, equipping them with the confidence and skills to move forward positively- even during difficult times.
